Agitation and Excitement Agitation and excitement are often the symptoms that lead to recognition of and hospitalization for exacerbations of schizophrenia. Historically, antipsychotics have been used both for these symptoms and for the psychosis, but a number of clinicians report that the SGAs seem less effective for the agitation and excitement of an acute exacerbation. For this reason, the algorithm for these symptoms is separate from the algorithm for psychosis and allows for PRN use of FGAs, benzodiazepines, olanzapine IM, risperidone oral solution or ziprasidone IM. It is important to stress that these PRN treatments should be time limited and discontinued as soon as clinically feasible. In the case of the FGAs this is because of increased risk of EPS, dysphoria, and tardive dyskinesia. In the case of benzodiazepines, the desirability of limiting amount and duration of PRN use relates to the development of tolerance over 23 weeks of steady use. On an outpatient basis, benzodiazepines should be used with caution in patients with a recent history of alcohol or drug abuse. Clinician choice of medication for agitation and excitement should be individualized to the needs and circumstances of the patient, guided by past history of response. Outpatients are likely to be more familiar with self-administering benzodiazepines on a PRN basis and may need education on PRN use of one antipsychotic while taking another regularly. Outpatients with a history of EPS should be started on an anticholinergic concurrent with starting a PRN FGA. Olanzapine IM.
In addition to their use in treating schizophrenia, atypical antipsychotics are widely used off-label to treat other psychiatric disorders. Although olanzapine is the only atypical with an additional indication, which is for acute treatment of bipolar mania, other atypicals are expected to gain indications soon for treating bipolar as well as other mental disorders, such as dementia. Risperidone is an example of an atypical antipsychotic that has been shown to improve symptoms of psychosis and aggression in severely demented patients Katz, 1999 ; , and preliminary evidence suggests it may be efficacious in treating conduct disorder in adolescents Findling, 2000 ; . Likewise, there is early evidence that low-dose ziprasidone may be comparable to diazepam as an anxiolytic, without producing a sedative effect Wilner, 2002 ; . In summary, the atypical antipsychotics offer improved therapeutic options for the treatment of schizophrenia. They are not interchangeable, having different pharmacologic, efficacy, and safety profiles. Overall, they work well in the treatment of positive and negative symptoms while offering a safety profile that is superior to that of the conventional agents in terms of reduced risk of EPS and TD. This combination of efficacy and safety results in improved therapeutic outcomes for patients and their families, along with healthier financial statements for health care systems. Methodology : Sixty adults 30 smokers and 30 nonsmokers ; , aged 40-70 years were studied. Swabs and Carbohydrate assimilation SDA, Corn Meal Agar, API20C AUX System ; were performed. Specimens were collected from dorsum of tongue, buccal mucosa and commissures right and left each ; . Colony forms were established by positive colony forming units, on SDA Medium 24-48 hours ; . Germ tube for true pseudohyphae ; growth, was done on Corn Meal Agar Medium. Candida biotype, is evaluated by API20C AUX System, which has a numerical 7 digit profile, added to evaluate a definite Candida biotype Results : 28.3% of Malaysian adults harbored Candida. Candida biotypes identified were as follows: C. albicans 59%, C. glabrata 17%, 6% each of C. famata, C. tropicalis, C. guilliermondii and C. lusitaniae. Seventeen positive cases, of these 53% were smokers and 47% non smokers. These consisted of Malays 40%, Chinese 33% and Indians 27%. Conclusion : Findings demonstrate that C. albicans is the most common biotype found in the oral cavity of Malaysian adults. It is equally prevalent in smokers and non-smokers, but showed a predilection for the ethnic Chinese group.

Finally, it has been suggested that a serotonin 5-HT1A receptor agonistic component may be beneficial in relieving the anxiety that can trigger psychotic episodes in schizophrenics.282 In summary, these findings suggest that compounds which combine dopamine D2 receptor antagonism with serotonin 5-HT1A receptor agonism may have enhanced antipsychotic activity and a reduced EPS liability. However, the dopamine D2 serotonin 5-HT1A receptor affinity ratio, as well as the degrees of intrinsic efficacy at these receptor subtypes required for an optimal clinical profile need to be established. Nevertheless, several compounds with the indicated pharmacological profile have recently been disclosed Chart 1.13 ; . Mazapertine RWJ-37796, 64 ; has high affinity for dopamine D2, D3, and serotonin 5-HT1A receptors Ki values of 2.2, 1.8 and 1.7 nM, respectively ; but also for 1-adrenergic receptors Ki 1.3 nM ; .310 At serotonin 5-HT1A receptors, it behaves as a partial agonist. The chromane derivative EMD 128130 65 ; has high affinities for dopamine D2-like D2: Ki 20 nM ; and serotonin 5-HT1A Ki 1 nM ; receptors only, the activity residing in the R ; enantiomer.25, 45 EMD 128130 behaves in vivo as a dopamine D2 receptor antagonist and a serotonin 5-HT1A receptor agonist. PD 158771 66 ; is the lead compound of a series of aminopyrimidine derivatives with high affinities for both dopamine D2, D3, and serotonin 5-HT1A receptors Ki values of 5.2, 13.7 and 3.5 nM respectively ; . At both dopamine D2 and serotonin 5-HT1A receptors, 66 behaved as a partial agonist.408 Recently several compounds have been disclosed, which in addition to dopamine D2 and serotonin 5-HT1A receptors, also possess high affinity for serotonin 5-HT2 receptors. The anthranilamide 1192U90 67 ; is the lead compound of a series of substituted benzamides with such receptor binding profiles.282, 283 Like ziprasidoen it bears a 4- 1, 2benzisothiazol-3-yl ; -1-piperazine moiety, which presumably is responsible for the high serotonin 5HT2 receptor affinity. A similar receptor binding profile was reported for the structurally closely related thiazolidinone derivative P-9236 HP-236, 68 ; .169, 170 Compounds 6468 were all active in animal models with predictive value for antipsychotic activity e.g. conditioned avoidance responding, inhibition of apomorphine-induced mouse climbing ; . Furthermore, evaluation in animal models with predictive value for EPS liability in man e.g. catalepsy, inhibition of apomorphineinduced stereotyped behaviour ; showed that they all are likely to have a low propensity to cause EPS. These results are promising and suggest that compounds with mixed dopamine D2 receptor antagonistic and serotonin 5-HT1A receptor agonistic properties may have potential as atypical antipsychotic agents.
