In addition to their use in treating schizophrenia, atypical antipsychotics are widely used off-label to treat other psychiatric disorders. Although olanzapine is the only atypical with an additional indication, which is for acute treatment of bipolar mania, other atypicals are expected to gain indications soon for treating bipolar as well as other mental disorders, such as dementia. Risperidone is an example of an atypical antipsychotic that has been shown to improve symptoms of psychosis and aggression in severely demented patients Katz, 1999 ; , and preliminary evidence suggests it may be efficacious in treating conduct disorder in adolescents Findling, 2000 ; . Likewise, there is early evidence that low-dose ziprasidone may be comparable to diazepam as an anxiolytic, without producing a sedative effect Wilner, 2002 ; . In summary, the atypical antipsychotics offer improved therapeutic options for the treatment of schizophrenia. They are not interchangeable, having different pharmacologic, efficacy, and safety profiles. Overall, they work well in the treatment of positive and negative symptoms while offering a safety profile that is superior to that of the conventional agents in terms of reduced risk of EPS and TD. This combination of efficacy and safety results in improved therapeutic outcomes for patients and their families, along with healthier financial statements for health care systems. [a]--The Daubert Standard In 1993, the United States Supreme Court in Daubert v. Merrill Dow Pharmaceuticals, Inc.11 clarified the grounds upon which an expert's opinion can be excluded or challenged in the federal courts. The narrow issue before the Court was whether the expert's opinion must be excluded if it is based on a scientific technique that is not "generally accepted" as reliable in the relevant scientific community. The Court ruled that the "general acceptance" standard was incompatible with the Federal Rules of Evidence and should not be applied in the federal courts. The court went on to point out, however, that under those Rules the trial judge must ensure that any and all scientific testimony or evidence is not only relevant, but reliable.11.1 The court noted that the trial court could consider the following factors: whether the theory or technique in question could be and had been tested; whether it was and griseofulvin. New generation antipsychotics are one of the fastest growing categories of drugs. "Atypical" because they treat anergy, apathy, and depression as well as overt psychosis with little or no EPS extra-pyramidal syndrome ; . These include risperidone Risperdal ; , olanzepine Zyprexa ; , ziprasidone Geodan ; , quetiapine Seroquel ; , and aripiprazol Abilify ; . Symbyax, the first FDA approved combined medication for bipolar depression, mixes olanzepine and the SSRI fluoxetine Prozac ; . Several SSRI's selective serotonin reuptake inhibitors ; introduced for depression, namely paroxetine Paxil ; and escitalopram Lexapro ; are now approved for anxiety and micronase. Objectives : To determine the various oral Candidal biotypes among healthy Malaysian adults in order to establish a database for local future references. Introduction : Candida is a normal commensal of oral mucosa. It is prevalent in diabetics, denture wearers, smokers and those with xerostomia. Methodology : Sixty adults 30 smokers and 30 nonsmokers ; , aged 40-70 years were studied. Swabs and Carbohydrate assimilation SDA, Corn Meal Agar, API20C AUX System ; were performed. Specimens were collected from dorsum of tongue, buccal mucosa and commissures right and left each ; . Colony forms were established by positive colony forming units, on SDA Medium 24-48 hours ; . Germ tube for true pseudohyphae ; growth, was done on Corn Meal Agar Medium. Candida biotype, is evaluated by API20C AUX System, which has a numerical 7 digit profile, added to evaluate a definite Candida biotype Results : 28.3% of Malaysian adults harbored Candida. Candida biotypes identified were as follows: C. albicans 59%, C. glabrata 17%, 6% each of C. famata, C. tropicalis, C. guilliermondii and C. lusitaniae. Seventeen positive cases, of these 53% were smokers and 47% non smokers. These consisted of Malays 40%, Chinese 33% and Indians 27%. Conclusion : Findings demonstrate that C. albicans is the most common biotype found in the oral cavity of Malaysian adults. It is equally prevalent in smokers and non-smokers, but showed a predilection for the ethnic Chinese group. Side effects of ziprasidoneFormulary update, from page 3 hypokalemia or hypomagnesemia, concomitant use of other drugs that prolong the QT-interval and the presence of congenital prolongation of the QT-interval. Siprasidone should be avoided in patients with conditions or concomitant drugs that prolong the QT-interval. In patients at risk of significant electrolyte disturbances should have baseline serum potassium and magnesium and imodium and ziprasidone. Ziprasidone clinical studies
