Appendix II 1. ASTRO PU APU ; Age, sex and temporary resident originated prescribing units Derived by the Prescribing Research Unit PRU ; in 1993, ASTRO-PUs were designed to weight individual practice populations for age, sex and temporary residents rather than just the patients aged 65 and over as in the PU weighting system. The ASTRO-PU figures used are based on cost, rather than number of prescription items although figures based on items have been produced. These item-based figures are now available in a table below ; . Cost-based ASTRO-PUs have subsequently been used by the NHS Executive and health authorities in prescribing allocation methodology and as the appropriate denominator when comparing the costs of prescribing between practices or between HAs. 2. STAR PU SPU ; Specific therapeutic group age-sex related prescribing units ASTRO-PUs were devised from the total of all drug costs. It is therefore not strictly correct to use NIC per ASTRO-PU for making comparisons within a therapeutic group e.g. NIC ASTRO-PU for ulcer-healing drugs. There are differences in the age and sex of patients for whom drugs in specific therapeutic groups are usually prescribed. To make such comparisons STARPUs have been developed along similar lines to the ASTRO-PU but based on costs within therapeutic groups. These were recently revised as STAR 97 ; PUs. 3. ADQ Average Daily Quantities To allow comparison of prescribing within England there is a need to have a system which more accurately reflects GPs' prescribing. Hence Average Daily Quantities ADQs ; have been developed by an expert group convened by the PSU. The following information is considered when defining an Average Daily Quantity; i. the Defined Daily Dose if one is available ; , as the World Health Organisation Advisory Group have much experience in this area and have access to a variety of data sources when defining values. However, it should be noted that DDDs are an `international' compromise and do not necessarily accurately reflect prescribing patterns in England. the Prescribed Daily Dose, if available and when calculated on a large enough sample of items, should also be considered, as it reflects the. Characterization of biotic and abiotic factors The physical, chemical, and biological characteristics of the orders of stream reaches studied are shown in Table 1. Higher water temperatures were recorded, as expected, in the rainy summer period. The temperature variation between sampled periods reached up to 10C. The waters were well oxygenated in both seasons. The pH data obtained suggested a slightly acidic environment with water values close to 7.0 during the dry season, except for the first-order stream reach. A positive correlation between pH and temperature was found r 0.73, P 0.05 ; during the rainy season. Higher dissolved organic carbon concentrations were observed in the rainy season. Total nitrogen and total phosphorus concentrations showed no limitations of these essential nutrients in the study area. No clear correlation was detected in Chl a concentration between the two seasons in the different orders of stream reaches. Aquatic enterobacterium identification A total of 111 bacterial isolates 45 from the rainy season and 66 from the dry season samplings ; were identified to at least the genus level: Citrobacter sp N 2 ; , Enterobacter sp N 9 ; , Edwarsiella sp N 4 ; , Escherichia sp N 8 ; , Klebsiella sp N 52 ; , Kluyvera sp N 1 ; , Morganella sp N 16 ; , Proteus sp N 5 ; , Providentia sp N 2 ; , and Serratia sp N 12 ; Antimicrobial resistance The percentage of freshwater enterobacteria showing antimicrobial resistance was determined, and resistance was demonstrated for all antimicrobials tested and mercury bichloride. Of the total 102 enterobacteria analyzed nine were excluded since they belonged to genera with N 5 ; from seven orders of stream reaches, 93% showed resistance to at least one antimicrobial at some level Table 2 ; . Kluyvera was the only genus susceptible to all antimicrobials tested, because volyaren long term. A single course of rituximab with concomitant methotrexate therapy provides significant improvements in disease activity in patients with active, longstanding rheumatoid arthritis RA ; who have had an inadequate response to one or more anti-TNF therapies, according to the results of this randomised, double-blind, controlled trial. Patients were randomised to receive intravenous rituximab n 311, one course consisting of two infusions of 1, 000mg each ; or placebo n 209 ; , both with background methotrexate. The primary efficacy endpoint was a response on the American College of Rheumatology 20% improvement criteria ACR20 ; at 24 weeks. Secondary endpoints were responses on the ACR50 and ACR70 improvement criteria, the Disease Activity Score in 28 joints and the European League against Rheumatism response criteria at 24 weeks. The following results were noted for the intention-to-treat population: The majority of the patients enrolled had an inadequate response to at least one previous anti-TNF agent approx 90% ; . 51% of those assigned to rituximab achieved ACR20 vs. 18% of those who received placebo infusions p 0.0001 corresponding results for achievement of ACR50 and ACR70 were 27% vs. 5%, and 12% versus 1%, respectively both p 0.0001 ; . Patients who were randomised to rituximab had `clinically meaningful' improvements in fatigue, disability and health-related quality of life.
