Reading et al8 and Giladi et al9 have shown large improvements in cognitive functioning with rivastigmine. Bergman et al10 and Fabbrini et al11 have found improvements in psychotic symptoms with donepezil. Donpezil Two very small, placebo-controlled and blinded studies have shown benefits on cognition with donepezil.12, 13 Another study by Brashear et al14 gave mixed results. This was a small, 12-week, double-blind, placebo-controlled, single-site study on donepezil, followed by a 33-week open-label study. In the double-blind phase, donepezil-treated patients had an improvement of 0.7 on the Mini-Mental State Examination MMSE ; , and a worsening of three points on the Geriatric Depression Scale GDS ; . There were no differences on CDR items. This small study suggests that donepezil has limited efficacy in PDD. It was also suggested that PD symptoms as measured by the Unified Parkinson's Disease Rating Scale [UPDRS] ; worsened in patients taking donepezil. Rivastigmine Emre and associates15 conducted a large, doubleblind, randomized, placebo-controlled, parallelgroup, multicenter, multinational study of rivastigmine in 541 patients with PDD. There was no significant difference from baseline in either the rivastigmine or placebo groups on the UPDRS; thus, parkinsonian symptoms were not worsened by rivastigmine treatment. There was a two-point reduction on the ADASCog in the rivastigmine group, compared with a one-point increase in the placebo group P 0.001 ; . In a 24-week categorical analysis of ADCSCGIC, more patients taking rivastigmine were clinically assessed as having improved and more patients on placebo were assessed as having worsened. At 24 weeks, rivastigmine patients had decreased one point from baseline on the ADCS-ADL scale, compared with a decrease of 3.5 points for placebo. Patients taking rivastigmine had a decrease of two points on the NPI, compared with no change in the placebo group P 0.015 and arimidex!
Drying oils like linseed, walnut and poppyseed have been widely used as binding medium for paintings. When fresh, these viscous oils consist of relatively apolar mixtures of triacylglycerols TAGs ; with different fatty acid profiles. Only a small percentage of these fatty acids FAs ; is fully saturated 6-9% ; whereas most of the other FAs are polyunsaturated 68-90% ; [292]. This high amount of unsaturated FAs, highly susceptible to oxidation, makes it possible for the oil to form a dried film. In this case drying is not the evaporation of a solvent but chemical drying, which involves the cross-linking of TAGs to high molecular weight substances [124, 295]. Due to the chemical environment in paintings, these "binding" substances are not stable end products. Further chemical changes in the oil paint film involve hydrolysis of the ester bonds, formation of new oxygen containing functional groups, oxidative cleavage of the fatty acid hydrocarbon chains, and metal ion coordination of the fatty acid group of the cross-linked material and non cross-linked fractions. A relatively good understanding of the process of curing exists as a result of detailed chemical studies of alkyd paint [123, 125]. The transition of the initial "polyester" to other forms of polymeric systems in ageing traditional oil paint is less understood. It is likely that the different processes partly overlap in time. On the basis of literature on ionomers and linseed oil based ceramer coatings it has been speculated that the polymer end groups react with or are partially adsorbed on the inorganic particles [262, 278, 379]. As part of our studies on the formation of ionomeric systems in historical paints, in this study a new analytical strategy is developed to monitor the process of de-esterification of the initial TAGs of oil paint systems. A first description of these processes and preliminary chemical data have been reported by Boon et al. [28] and Van den Berg et al. [29]. Already since the `60s oil paint samples have been analysed using a combination of chemical work-up and subsequent gas chromatography analysis [10]. Initially this was done to identify the type of oil used for painting based on the determination of the ratio of saturated palmitic- and stearic acids which, at that time, were thought to remain unchanged within the paint film upon ageing. First, the paint sample was completely hydrolysed in order to analyse liberated oxidised ; fatty acids. After extraction into an organic layer, a washing step and acidification the fatty acids were methylated and analysed. Later on conservation ; scientists became interested in the way the fatty acids are present within the paint film [22]. After immersion of the paint sample in an organic solvent the extractable fraction, consisting of free fatty acids, acylglycerols and smaller cross-linked networks, were separated from the metal salts and high molecular weight materials which were thought to be insoluble. Both fractions were analysed as previously 172.

Although gastrointestinal events were generally of mild to moderate severity in patients receiving 6 mg to 12 mg of rivastigmine daily in placebo-controlled trials, such events occurred relatively frequently, particularly during forced weekly dose titration.67, 68 Although similar patterns were observed with forced titration schedules for galantamine and donepezil, it has been proposed that slow-dose escalation ie, at 4-week intervals ; and delayed absorption by administration with food may be particularly important with rivastigmine, as its pharmacologic properties may lead to relatively potent and fast elevation of ACh levels.90 Cardiovascular adverse effects, such as a bradycardia, can occur in association with cholinesterase inhibitors. This can be particularly problematic for patients with underlying supraventricular cardiac conduction disorders such as sick sinus syndrome. In support of this hypothesis, selective central cholinesterase inhibition may be associated with reduced probability of peripheral cholinergic adverse events.91 For example, a pooled analysis of placebo-controlled trial data showed that rivastigmine, 6 mg to 12 mg per day, was not associated with a significantly increased incidence of cardiovascular adverse events, heart rate or blood pressure changes, or any abnormalities in the results of electrocardiograms, including vagal effects.91 Neither bradycardia nor syncopal episodes were reported as adverse events in placebo-controlled trials of rivastigmine or galantamine in patients with AD.67, 68, 7476 Prescribing information acknowledges that, in placebo-controlled trials, syncopal episodes have been reported in 1% to 2% of placebo recipients and in 2% to 3% of subjects receiving rivastigmine, 6 mg to 12 mg per day, or galantamine, 24 mg per day. Placebo-controlled trials with fonepezil have reported bradycardia or syncope as adverse events in association with 1% to 3% of subjects receiving dknepezil and up to 2% of placebo recipients. Prescribing information warns that syncopal episodes have been reported in association with the use of donepezil. Neuromuscular adverse events, such as muscle cramps, can occur as a result of excessive stimulation of nicotinic receptors at the motor endplate. It has been suggested that selective inhibition of the G1 form of AChE, which is not prevalent in skeletal muscle, may decrease the risk of muscle cramps or weakness.32 In pivotal trials, the incidence of muscle cramps was reported as rare in association with high-dose rivastigmine or galantamine and no different from that reported for placebo.67, 68, 7476 In their pivotal placebo-controlled trial, Rogers and colleagues61 reported muscle cramps in significantly more subjects treated with donepezil, 10 mg per day, than in placebo recipients 8% vs 1% of subjects, respectively; P .05 ; . Insomnia has been reported in association with conepezil in placebo-controlled trials.89 If donepezil is administered before retiring, peak plasma concentrations will occur during sleeping hours.92 Therefore, morning administration may eliminate sleep disturbances.92.

We have found some analogies but also some differences in the extent of neuroprotection as well as in the mechanism involved, among galantamine, donepezil, rivastigmine, and tacrine.
