Clinical sharp objects reported as enterovesical foreign bodies in humans might have. The stick perforated the small intestine, and caused a focal peritonitis and adhesions that attached the bladder to the jejunum. The stick may then have been pushed into the bladder by the peristaltic movements of the small bowel. It is also possible that the bladder has actively enveloped the stick once it entered the peritoneum. The reports of intravesical migration by objects such as gauze suggest that the bladder has a natural ability to do this. On presentation, the dog was azotaemic with urine specific gravity 1.020, consistent with renal failure. Mature neutrophilia and hypoalbuminaemia were also present secondary to severe inflammation, protein effusion into the bladder14 and decreased production of albumin due to increased hepatic production of acute phase reactants.15 Ultrasonic examination showed changes consistent with hydronephrosis secondary to urinary obstruction, which was supported by the finding at surgery of bilateral dilatation of the ureters. The chronic nature of the partial urinary obstruction appears to have led to permanent renal insufficiency, as repair is largely complete 2 to 3 months following acute renal injury.16 An obstructed infected bladder placed the patient at high risk of pyelonephritis.17 Renal pelvis urine was not aspirated for culture to confirm renal involvement. It is recommended that animals with pyelonephritis be treated with antimicrobial drugs chosen on the basis of urine culture and sensitivity, that urine cultures be repeated during therapy, and that the antimicrobials are given for an extended time usually 4 weeks or longer ; .17 These recommendations were followed in this case. The indwelling catheter placed after the operation may have let more bacteria into the bladder, but it kept the bladder from being distended and enabled monitoring of urine volume to detect oliguria in a patient at risk of acute renal failure, thus improving the capacity of the bladder to recover.18 The catheter was maintained as a closed system by sealing a sterile container to its distal end. Organisms were still able to migrate along the outside of the catheter, but they were exposed to mucosal defences, which may have reduced the numbers that ultimately reached the bladder. The initial clearing of the urinary tract infection, only to be complicated by a new infection while the patient was still receiving amoxycillinclavulanate, was due to several factors. The urinary tract defends itself via mucosal defences, anatomical structure and antibacterial properties of urine.19 The foreign body and its surgical removal decreased mucosal defences by disrupting the surface glycosaminoglycan layer, decreasing immunoglobulin A production and impairing intrinsic properties of the endothelium secondary to replacement by fibrous tissue, as well as decreasing the function of the vesical musculature and the urethral high-pressure zones. Urine itself has antibacterial properties, attributed to factors such as concentration of urea, and osmolality. ARTHRITIS PRESENTING DURING THE ACUTE PHASE OF KAWASAKI DISEASE Grace WK Gong, Joyce C Ching, Brian W McCrindle, Rae SM Yeung Hospital for Sick Children ; Objective To define the incidence, pattern and clinical course of arthritis presenting at time of diagnosis of Kawasaki Disease KD ; in children. Method A single center, retrospective study of 421 consecutive patients diagnosed with KD between 01 1997 and 12 2002 was performed. Standardized clinical assessments, laboratory, and imaging test results plus treatment regimens were reviewed. Coronary arteries were visualized using echocardiography and coronary artery lesions CAL ; were reported as BSA adjusted Z scores. Those presenting with arthritis were identified, and the pattern of arthritis and clinical course documented. Children were separated into two groups, those with arthritis and those without arthritis, and clinical, laboratory, treatment response, and coronary outcome data were analyzed. Results During the study period, arthritis was documented in 38 children versus 383 without evidence of arthritis. The children with arthritis had effusions in one or more joints at KD diagnosis. The pattern of affected joints was variable, with both pauci- and polyarticular patterns and involvement of both large and small joints. In the majority of cases, the arthritis rapidly resolved with standard therapy for acute KD and did not require any additional medications. The children presenting with arthritis did not differ from children without arthritis with respect to their demographics, clinical features or laboratory findings. Additionally, presence of arthritis did not alter the response to treatment with IVIG, nor coronary outcome. Conclusions Arthritis is common during the acute phase of KD. The pattern of joint involvement varies with both pauciand polyarticular involvement. Most cases of arthritis resolve without additional therapeutic intervention. Children with arthritis share all the same clinical features, biochemical parameters and coronary outcome as patients without arthritis. Acute inflammation of multiple targets, including joints, is part of the clinical spectrum of acute KD and azelaic. Diagnosis of thyroid disease Thyroid disease should be diagnosed using a combination of clinical symptoms and thyroid function tests TFTs ; .8, 9 If the TSH level is abnormal, T4 can be measured; if TSH concentrations are undetectable but T4 is normal, a T3 level is appropriate. All three are measured in difficult cases, where suspicion of thyroid dysfunction is high.4, 9 Indices have been developed that can help to determine the probability of thyroid disease being present, based on patients' symptoms.10 The possibility of false negative and especially false positive results should be borne in mind with TFT results of patients with a low probability of disease. Current consensus suggests that patients who present with symptoms that appear to be thyroid-related but who have normal TFT results, should not be treated.11, 12 Other causes of their symptoms should be investigated.
