These hearts were exposed to circulating concentrations of acetaminophen equal to 50 g ml. This is inside the range of therapeutic-to-safe concentrations described in the literature. Comparing results using controlled flow versus controlled pressure, a picture of modest but significant acetaminophen-mediated coronary vasoconstriction emerged. However, only the highest concentration 0.6 mmol l; 100 g ml in human plasma ; consistently produced this effect, and there were qualitative differences between the two modes of circulation. Our objective for this part of the study was to identify coronary vasoactive properties of acetaminophen. From these results, we cannot determine the mechanism of acetaminophen-mediated coronary vasoconstriction, nor if the effects were mediated directly at the vascular smooth muscle or at the underlying endothelium. We can only conclude that concentrations at the lower end of the safe-to-therapeutic range did not directly affect the coronary circulation, and those that did produced only modest, albeit statistically significant effects. To our knowledge, no one has reported the influence of acetminophen at 50 g the human heart and coronary circulation. At the highest concentration acetaninophen appears to have positive inotropic properties. Although this effect was influenced by the mode of circulation, the mechanical responses were not caused by the circulatory effects of the drug. If the circulatory effects had influenced mechanics, one would have expected negative inotropy in the face of a reduction in oxygen supply, i.e., acetaminophen-mediated coronary vasoconstriction. Conversely, LVDP in some isolated, perfused rodent heart preparations seems to be sensitive to coronary perfusion pressure. Hence, under conditions of controlled flow in such preparations, small- to moderate-increments in coronary perfusion pressure can indirectly cause positive inotropy. Ss UTILIZATION PATTERNS OF ACETAMINOPHEN APAP ; IN A MEDICAID POPULATION Kirkland K * , Daignault B, Parmar K. Horizon Mercy, 210 Silvia St., West Trenton, NJ 08628 PURPOSE: To identify and evaluate the prescribing patterns of acetaminophen APAP ; in an adult Medicaid population. METHODS: Two months of retrospective prescription claims for all Medicaid members were captured. Members identified as potentially having received a daily cumulative dose 7 grams of APAP in at least 1 instance were included in the review, and intervention letters and surveys were mailed to prescribing physicians. Data was analyzed postintervention to determine a decrease in total daily dose of APAP a reduction in the number of , physicians writing prescriptions per member, and the overall physician prescribing habits. RESULTS: A total of 123 members received an average maximum daily dose of 8.6 grams of APAP prescribed by 236 different physicians. These physicians were prescribing on average 13.12 APAP-containing prescriptions for all Horizon Mercy members. Postintervention, there was a 78.9% reduction from baseline in the number of patients receiving 7 grams of APAP per day, and their average maximum daily dose of APAP decreased to 5.5 grams. There was an 80.7% decrease from baseline in the number of physicians prescribing for these members. The overall prescribing of APAP by these physicians decreased by 30.9% from baseline. An estimated 31% of the physicians responded to the utilization review. A majority of the physicians were not aware that multiple physicians were writing prescriptions for their patients, stated they would alter their prescribing patterns for APAP, and categorized the information provided as "very useful."
